Our Backstory
After decades of progress, healthcare data is more available than ever, however it is still not made meaningful. Connective Health’s vision is to ensure every clinician understands the story of every patient in 30 seconds, with treatment support options at their fingertips. This has historically been a missing step in achieving more integrated care, and therefore better outcomes at a lower cost.
Today, the service provides a complete clinical picture across the patient’s care team as well as predictive insights and care collaboration tools. Connective Health’s technology is used by at-risk primary and specialty care providers as well as accountable care organizations to drive better health outcomes at a lower cost. The company was founded by a veteran team of healthcare interoperability innovators.

Chris Diblasi
Chris has spent his career delivering technology solutions and is passionate about advancing healthcare. He held product roles at Surescripts and Optum focused on interoperability, value based care, and clinical operations. He also served as an advisor to technology executives at Booz Allen Hamilton for seven years.

James Bradley

Paul Uhrig
Paul is currently the Chief Legal & Digital Health Officer at Bassett Health. Previously, Paul heldseveral C-Suite and board member roles in the healthcare policy, privacy and security field. Notably he was Surescripts’ Chief Administrative, Legal, & Privacy Officer and for a period its Acting CEO.

Dr. James Taylor
Jim is a board-certified family physician and certified coder. His leadership activities include serving on Kaiser Permanente’s National Revenue Cycle board, co-chairing Kaiser’s ICD-10 Compliance Committee, and chairing the 1,200 physician Colorado Permanente Medical Group. He served as National Medical Director for Iora Health, assisting in starting their MA program and served as National Medical Director for Humana’s Partners in Primary Care.

Dr. Mehdi Qalbani
Mehdi is a board-certified physician with 20+ years of experience in behavioral health, substance use disorder, and telepsychiatry. He is currently the Psychiatric Medical Director for Upward Health and previously the Founder and Managing Partner of Integrated Behavioral Health, now part of Optum.

Dr. Joel Diamond
Joel previously served as CMIO and chairman of the Physician Advisory Board at the University of Pittsburgh Medical Center, St. Margaret Memorial Hospital. He was the cofounder of dbMotion, a health data solution acquired by Allscripts. He is currently an adjunct professor at the University of Pittsburgh, diplomat of the American Board of Family Practice and fellow in the American Academy of Family Physicians. He continues to treat patients at the Handelsman Family Practice.
